Welcome to the Aging and Disability Partnership ToolKit on Managed Long Term Services and Supports
Like many areas of the nation’s health care system, community-based aging and disability organizations (CBOs) are experiencing significant change. With change comes uncertainty, and for many of us, that uncertainty takes us out of our comfort zone.
The purpose of this toolkit is to help CBOs learn about, understand, and prepare for success during this change and provide you with tools that you and your leadership team can use to get your CBO ready to expand the ways you do business and enhance your ability to help current and future clients live healthy, independent lives in the community through managed long term services and supports (MLTSS).
CBOs across the nation are at different stages of development and experience working with the private sector. Some will find this to be a refresher while others will be new to the process. Both are wonderful places to be and we encourage you to explore and provide feedback so the A/D Partnership can continue to improve and expand the toolkit to meet the needs of CBOs in all phases of reform.
We’re working with CBO leaders, business consultants, managed care organizations, and ACL to rollout modules that address the most frequently requested topics.

Getting Started: If you are a traditional CBO and operate exclusively or near exclusively from grants, and government funding projects, that’s great, you’ve come to the right place! You are not alone! The “Getting Started” module is for you. Even if you have done some work offering services to hospitals, managed care companies, employers, or other individuals or entities on a private pay basis, this is still a good place to start.
Assessments: This module provides two assessment tools to guide you in your planning. Use this module for self-assessment and market-assessment tools and information.
Resources: Lists and Links to valuable resources, including performance measures, definitions, assessment tools, and recommended reading.
Value Proposition, Pricing and other Financial Considerations, and additional modules are under development and will be added in the coming months.
This Toolkit is a product of the Aging and Disability Partnership (A/D Partnership) as part of the Administration for Community Living (ACL) grant “Building the Business Capacity of Aging and Disability Community-Based Networks for Managed Long-Term Services and Supports”.
The A/D Partnership was established by the National Association of Area Agencies on Aging (n4a) and includes the National Disability Rights Network (NDRN), the Disability Rights and Education Defense Fund (DREDF), the National Senior Citizens Law Center (NSCLC), and Health Management Associates (HMA).
